Velos Access Request Form

Complete this form if you are requesting a new user account, modifying existing access, or requesting that your account be reactivated. To be granted access to Velos eResearch, you must be a UM employee, student, associate, or research affiliate, who is active on a study team or fits another defined usage category AND has completed the mandatory training requirement. Training requirements can be found at http://uresearch.miami.edu/orim/

All fields are required in this form. Complete each portion and submit the request. You will receive a document with required signatures, once you complete the signature send it to ovprshelpdesk@miami.edu for further review. Once all requirements are met, you will receive an email with your access credential as well as your unique e-signature.
