Our Pain Management Program at Boro Clinic is designed to provide comprehensive, integrative care for patients dealing with chronic pain. By enrolling in this program, patients can access a team of healthcare professionals dedicated to managing and improving their quality of life.
Services Included:
Access to your care team, including primary care physicians, case manager, via phone, email, and other non-face-to-face communication methods.
Routine appointments with your designated pain management provider or a member of your care team to monitor and adjust your pain management plan.
Comprehensive management of pain-related conditions, including:
Creation of a personalized pain management plan, reflecting your specific needs, values, and treatment goals.
Medication management and reconciliation to ensure safe use of pain-relief medications and other prescribed treatments.
Coordination of care as you transition between specialists, emergency departments, or after hospital discharge.
Referrals to pain specialists, physical therapists, or other relevant healthcare providers.
Access to integrative services, such as physical therapy, acupuncture, or behavioral health support, aimed at addressing the emotional and physical aspects of chronic pain.
Terms and Conditions: I understand that participation in the Pain Management Program includes the services described above and is intended to provide continuous, coordinated care for my chronic pain condition.
I acknowledge that:
I can revoke this agreement at any time, effective at the end of a calendar month, should I decide to pursue pain management services elsewhere.
Fees or copayments may apply based on my insurance coverage, for which I am responsible.
My designated provider may communicate electronically with other treating healthcare professionals to coordinate my care.
By signing this agreement, I designate Boro Clinic as my primary care provider for purposes of the Pain Management Program. I consent to the use and sharing of my medical information with relevant healthcare providers to ensure proper coordination of my care.
